NDIS Appointment

How long will an initial consultation take?

 The assessment consultation will take up to 1 hour and includes a review of the patient's background, medical history and a physical assessment.

 

Initial Consultation information requirements?

 The following information is required,  the NDIS participant's full name, Date of birth, NDIS number and the start and end date of their NDIS plan. 

I also need to know if the participant requires a quote only OR quote & assistive technology form template completed. Fees apply for the consultation and assessment report. Contact me for more information.

What do I need to bring along to the initial consultation?

The participant is requested to bring any referrals and medical history and or X-rays related to the individual's condition as well as any orthoses is currently use or have used to the appointment. 

What is an AT template?

An AT template is the Assistive Technology (AT) form that justifies the prescription of the orthotist and why it is necessary and required by the participant. 

The Assistive Technology template includes : justification for the orthoses, background, goals relating to the orthoses, condition deficiency, how the device addresses the deficiency, functional assessment report, achievement of goals and the outcome measures. 

The completed assessment report (AT Form) will be submitted to the NDIS once completed.

What happens after I receive the AT form?

The clinician will forward the completed AT form to the participant or their representative, the documentation then needs to submitted to the NDIS. The NDIS will then process the submitted AT form.

What happens once the NDIS assessment report & quote has been approved?

I will contact the participant or their nominated contact to schedule the appropriate appointment, once the service booking has been added to my NDIS service provider portal.
