the different NDIS funding plans

  The different NDIS funding plans :

1: NDIS (Agency) managed

2:Third Party (Plan managed)

3: Self managed

 If the participant is NDIS (Agency) managed, then prior to seeing the participant, a service booking on the NDIS portal for New Horizon BLS for the following hours, consultation (1 Hour)  and if required (3 Hours) for the completion of the assessment report and  associated documentation needs to created. This is done by the NDIS with the permission of NDIS participant. 

 If the participant is Plan managed by a third party, an email request from the agent is required containing a purchase order number OR guarantee of payment for the consultation and assessment report. This is required prior to seeing the participant.  

 If the participant is Self managed an invoice will be provided to the participant on the day of the consultation, the participant will be charged the consultation fee as well as the cost of the assessment report if required. 

Please note that for self managed NDIS plan participants, a 50% deposit is required prior to commencing manufacture or ordering of the required products. This deposit is additional to the consultation and if required assessment report fees.
